Intake Form

Important Note: • Please fill out the form honestly and thoughtfully; your responses will guide the coaching process. • Share specific challenges, goals, and expectations for better understanding. • All information provided will remain strictly confidential. • You will be giving 8-13 sessions to solve your problems as per coach suggestions • Per session of 45-60 minutes, and fee Rs. 5000/- will be charged for each session. • Mention any prior parenting courses, counseling, or relevant experiences. • Complete one form per parent/guardian participating. • Be open to learning, reflection, and change. • Submit the form at least 24 hours before your scheduled session. • Keep a copy for your reference.
